We do everything in our power to make your child’s appointments comfortable and positive at Pediatric Dental Associates, PC in Fort Wayne, IN. Dr. Hobbs and his staff will greet you and your child to begin establishing a relationship before treatment. You are invited to wait in the waiting room while we provide treatment, so we have your child’s attention and cooperation.

Please download and print out the patient forms below and bring completed copies to your child’s appointment. This helps us prepare to provide appropriate treatment for your child.
